How do I set up the reply document to carry over the values of the fields (e.g. category and subject) in the parent document when I create a reply document?

Please set the formula (@function) to the [Default Value in New Document] of the Form Info.
A sample is described below, and you can arrange it accordingly.

If you set the following as the default value in [Default Value in New Document] of "Subject" field in the "Reply" Form Info, the value with "Re: " at the beginning of the parent document subject will be set as the initial value.
"Re: " + @GetField( "Subject" )
